I acknowledge the potential risks involved in cheerleading activities and voluntarily assume all such risks. I hereby release, waive, discharge, and covenant not to sue 828 Elite Cheer Inc., its coaches, volunteers, sponsors, and facility owners for any injuries, damages, or losses incurred during participation in practices, competitions, or events. I understand that signing this waiver helps protect the organization and participants by making families aware of potential risks.

In the event of illness or injury, I authorize 828 Elite Cheer staff to seek medical treatment for my child/ward and to share medical information with healthcare providers as necessary. I agree that our family is responsible for all medical expenses associated with participation.

I give 828 Elite Cheer permission to use photographs, video footage, voice recordings, or written testimonials of my child/ward taken during practices, competitions, or events to promote the organization’s programs in print, online, and social media. I understand that neither I nor my child will receive compensation for such use, and that the images may be used at 828 Elite’s discretion.

I agree to pay the non‑refundable registration fee and any subsequent program fees according to the schedule provided. I understand that missed practices or voluntary withdrawal do not entitle me to a refund unless otherwise stated in 828 Elite Cheer’s refund policy.
